What is My McLaren Chart?

My McLaren Chart is a new feature our team of health care professionals is pleased to introduce for patient use.  It is available through the Internet to give you flexibility and help you manage your health information between visits. 

My McLaren Chart provides patients with access to health information online and anytime from any location using a computer, tablet or smart phone device. 
Health information available through My McLaren Chart includes:

  • Lab and radiology results

  • Visit history

  • Condition/Reason for visit

  • Care instructions

  • Team of care providers

  • Allergies verified by a health care professional

  • Home medications and medications prescribed upon discharge

  • Instructions for taking medications 

How do I enroll in My McLaren Chart?

  3. Complete enrollment questionnaire, (i.e., name, date of birth, email address and medical record number (this is a required field).  Please look for your medical record number on your discharge information forms if your nurse shared information about My McLaren Chart with you directly. 

  4. Access your email for your one-time username and password.  Click on the link in the email to return to My McLaren Chart.

  5. Enter your one-time username and password, enter your preferred, permanent username, password, answer security questions and click Log On.

  6. Enter your new username and password.

  7. Explore – anytime, anywhere!

Why has McLaren Port Huron created My McLaren Chart?

McLaren Health Care and McLaren Port Huron have been using an electronic medical record system for many years. An electronic medical record is a national standard of health care practice to enhance quality, safety, and efficiency by following consistent documentation guidelines. Ever-evolving technology requires our informatics professionals, clinicians and physicians to work together to create and update electronic record-keeping systems to benefit our patients. 

Where does my health information in My McLaren Chart come from? 

All of the information comes from your electronic health record at Port Huron Hospital.  It’s always up-to-date and accessible whenever you need the information – 24/7.

How do I log in to My McLaren Chart?

If you do not have a user name and password, please follow the enrollment instructions above under 'How do I enroll in My McLaren Chart?'

Is my information safe?

Yes, you are the only person who can access your health information on My McLaren Chart. Your health information is not stored on the Internet; it is retrieved from the application as pages are accessed.  As added security, a timeout feature protects your information if you leave the page open for an extended amount of time.  In addition, My McLaren Chart passwords are encrypted and URLs are re-written so that they cannot be copied and pasted.

You may give a family member permission to view your health information. However, consent is required to give your family member(s) access. Consenting parties must fill out the necessary forms.
