Location at Port Huron Hospital: Health Information Services is located in the Center Lobby across from the elevators at the "Health Records" desk. Notary of Public services are also available at this location.

Medical records may only be requested by the person they belong to or their legal representative according to HIPAA Federal laws.

There could be a fee for copies of Medical Records. Medical records may not be immediately available. Please allow seven business days for requests to be completed.

What is needed for your health records to be released?

  • Driver’s License or Picture ID MUST be presented

  • If you are sending someone on your behalf to pick up your records, you will need to make mention of this person’s name in the area on the Authorization for Release of Information and make sure to sign the Authorization for Release of Information giving Port Huron Hospital permission to release your records to this person .  The link to the Authorization for Release of Information is located above on this page in “blue”,   just click on it and print the authorization.  The designated person must bring photo identification in order for them to be released.

Medical Record Copying Services:

Port Huron Hospital has contracted HEALTHPORT to do its copying of medical records. This national copy company is fully insured and licensed to handle confidential information. Any questions about the status of your request can be directed to the HEALTHPORT REPRESENTATIVE at 810.985.2694.  Please leave a voice mail message if the Copy Representative does not answer.  Someone will return your call.

Notary Public, Monday through Friday from 8:30 a.m. to 3:30 p.m.

Notary of Public services are available to patients, visitors, Port Huron Hospital associates and medical staff members.

  • Bed-side Notary of Public services available to patients by calling extension 3127 for an appointment.  Please leave message if no one answers.

The following are rules that an individual seeking Notary Public services and the Notary must follow:

  1. The Signer MUST appear in person before the Notary Public at the time the instrument is being notarized
  2. The signer must provide identification either by:
    • Personal knowledge of the signers identity
    • The oath of a credible witness
    • Reliable identification documents with picture
  3. The Notary will not notarize an instrument if there is any doubt at all about the willingness or awareness of the signer.
  4. The Signers signature will be compared with the signatures on any other documents presented.
  5. There should be no blank documents or blank spaces left in the document by the requestor/signer.
  6. The Notary will review the document for missing information.
  7. The Notary will check all dates and will refuse any request to predate or postdate their Notarization.
